You can request to register online with the following link https://gp-registration.nhs.uk/M86617/gpregistration/landing. People under 16 can only be registered with a parent at this surgery.
You can also request to register by completing a registration form (GMS1) which can be downloaded here with associated information (EXTERNAL LINK), or directly here and returning it to the surgery. We do not keep physical copies of registration or any other documents.
New registrations can take up 3 days to update clinical systems and up to 15 days for NHS to authorise. Please enter all data on form carefully to avoid delays.

When you register, we will ask you bring a photo ID (eg passport or driving liscence) and proof of address eg. a utility bill. If you have not registered in the UK with a surgery before, you will need to complete a health check questionaire and make an appointment to see the practice nurse (or doctor if you are on regular medication) for a new patient health check. This will ensure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
