People under 16 can only be registered with a parent at this surgery. If you have any "shared care" agreement with an external organisation (eg hospital), please make us aware as we may not be able to continue registration and you will need to consider another surgery.
You can also request to register by completing a registration form (PRF1) which can be downloaded here with associated information (EXTERNAL PDF LINK), and returning it to the surgery. We do not keep physical copies of registration or any other documents.
New registrations can take up 3 days to update clinical systems and up to 15 days for NHS to authorise. Please enter all data on form carefully to avoid delays. Please order at least one month of any medications before requesting to register with us.

When you register, we will ask you bring a photo ID (eg passport or driving licence) and proof of address eg. a utility bill. If you have not registered in the UK with a surgery before, you will need to complete a health check questionnaire and make an appointment to see the practice nurse (or doctor if you are on regular medication) for a new patient health check. Please make us aware of any immunisations you have had outside England. This will ensure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
